Privacy Policy

Last updated August 2026. This Privacy Policy explains how Eitan Sheena M.D. PLLC ("we," "us," "our") collects, uses, and protects personal information when you visit this website, request an appointment, or otherwise communicate with our office.

Information we collect: contact details you provide, such as your name, phone number, and email address; information submitted through appointment or contact requests; and clinical information shared as part of your care, which is protected separately under HIPAA (see the HIPAA Notice below).

How we use it: to respond to inquiries, schedule appointments, communicate about your care, and meet our recordkeeping and legal obligations. We do not sell your personal information.

Cookies: this website uses only essential cookies required for the site to function, including a cookie that remembers your cookie preference. We do not currently use tracking, analytics, or advertising cookies. If that changes, this policy will be updated and you will be asked for consent again.

Changes: we may update this policy from time to time; updates will be posted here with a revised date.

HIPAA Notice

This notice describes how your health information may be used and disclosed, and how you can access it. Your privacy is protected under HIPAA, and your health information is generally used only to provide, coordinate, and bill for your care through Eitan Sheena M.D. PLLC.

In limited circumstances permitted by law, information may be shared without your authorization — for example, to comply with a court order, to prevent a serious threat to health or safety, or as otherwise required by law. For any other use, your written authorization will be requested first.

You have the right to request access to your medical record, request corrections, and request a list of certain disclosures. To make a request, contact our office in writing.

Practice Policies

Telehealth: telehealth appointments are offered alongside in-person visits, and Dr. Sheena can accommodate in-person evaluations directly. If your care requires a higher level of support than this practice provides, a referral to an appropriate provider or clinic will be offered.

First appointment: your first visit is a clinical consultation only, and does not itself establish a treatment relationship or a doctor-patient relationship. During this visit, Dr. Sheena will complete a comprehensive evaluation and share his diagnostic impressions and recommendations. If you and Dr. Sheena agree that ongoing care together is the right fit, you'll be formally accepted into the practice and treatment will begin. If not, you'll be provided with a referral to a more appropriate level of care.

Medication management: prescriptions, including refills, require an active treatment relationship and regular follow-up visits. Patients who have not been seen within the practice's standard follow-up window may need a new evaluation before medications can be renewed.

Cancellations: appointments cancelled with less than 24 hours' notice, or missed without notice, are billed at the full session rate.

Continuity of care and discharge: Dr. Sheena may end treatment with a patient under certain circumstances, including: three consecutive missed appointments; a lapse of three months or more without contact; a determination that a higher level of care is clinically indicated; a violation of practice policies; incidents of fraud; or transference and countertransference issues that interfere with effective treatment. Except in urgent situations, Dr. Sheena will provide written notice of discharge, continue care for a reasonable transition period, and offer referrals to help ensure continuity of care.
